Dr.Song has the vision of creating a medical group providing the best care to people in Boston area. This organization will have special interests in improving medical care for seniors in long term care and Korean immigrants in Boston .

Frailty Management is a geriatric consultation service, founded by Dr.Min Keun Song, MD, MPH, CMD, HMDC, geriatric specialist. We provide patients with their frailty index score based on the cumulative deficit model of frailty. We calculate the frailty index from routinely collected items of a comprehensive geriatric assessment. Based on the frailty index, we propose guided geriatric coaching to educate the customers and to empower self management of their conditions . For the patients with high frailty index, we coordinate proper interventions and treatments to prevent poor outcomes.

Dr.Song founded the delirium task force at BI-Milton hospital and has been working with physician colleagues, nursing leadership and hospital leadership in order to develop effective delirium prevention and management intervention.

Dr.Song has served multiple LTC facilities as medical director. He provides NH administrators and directors of nursing with guidance and recommendation regarding resident care, healthcare related regulation, facility policy & procedures. He also advocates the resident's rights compassionately.

Dr.Song serves Ascend Hospice as medical director. Dr.Song and Ascend Hospice team are dedicated to helping patients with terminal illness and their families manage their difficult time with dignity and love through attentive, individualized programs, services and alternative therapies.

Dr.Song is a Harvard trained and board-certified geriatrician. He provides geriatric primary care as well as geriatric consultation service at the clinic. His clinic is located at Beth Israel Deaconess Hospital in Milton (100 Highland Street, Suite 105, Milton, MA 02186).

Dr.Song performs gospel music at nursing homes in order to entertain nursing home residents and to support their faith.
